INSTRUCTIONS

To the pupil:

A complete application includes the following: 
(I) Pupil Application form  
1. Complete Section A and Section B of this application form. 
2. Please print out pupil contract & parental consent from the Downloads section below and submit the signed document to the teacher advisor of LDP in your school.

(II) Teacher Recommendation form 
To be completed by your chosen teacher referee.

You may approach any teacher in your school (subject or CCA) who knows you well to write your recommendation. Indicate to this teacher your wish to be in the LDP. Confirm with the teacher that he/she will be writing a reference in support of your application. Please let the teacher know that the on-line login details for writing the reference are with the HOD and LDP teacher-advisor of your school.
